Payments & Fees
At the start of each new dance season there is a registration fee for all participating families. The registration fee for the 2019-2020 dance season is $40 per family. We also have a $15 fee per person for any student taking an aerial dance class. This is to cover some of the extra expenses involved in apparatus maintenance and replacement.
When you register online at you will be able to set up a student account where you can make payments of any amount at any time during the year. This is our preferred method of payment. If you need to pay with cash or check, you may place in our drop box located next to the lockers in the small studio.
If you are dropping off a payment when the studio is closed, please use the mail slot in the door at 125.
Checks are to be made payable to Formations Dance Center. We charge a service fee for checks that are returned- $25 per check.
Tuition is due on the 1st of each month and is non-refundable. If the studio is closed or your dancer misses class, tuition is still due on the 1st of each month. If you have not paid your tuition by the 10th of the month, a $10 late fee will be added to your bill. If your account is not paid by the last day of the month, you will be sent a notice and accrue an additional $5 fee. Any family who has an overdue bill for more than 40 days will not be allowed to take part in class and will be asked to sit and observe. Notices will not be sent until one month overdue. Timely payment is much appreciated! Thank you.
Please note that once tuition payments, costume fees, recital fees, dancewear payments or registration fees are received, NO REFUNDS WILL BE GIVEN.
Questions or Concerns
If you need to speak with one of our instructors in person to address an issue or concern, please send an email to Miss Leslie to set up an appointment at email@example.com. In order to keep the flow of classes and the schedule running on time, we will be unable to take class time to speak with parents concerning student accounts or address any issues.
Most calls, texts, emails and messages will be answered M-F 9am-3pm. If a message is received when classes are in session, we may not be able to get back to you until the following business day. Thank you for your understanding.
* Students will be expected to arrive to class on time with their first pair of shoes on and ready to go.
* Please use the restroom and/or get a drink BEFORE class starts so that you will not have to miss class time to do so.
* No gum allowed.
* No jewelry will be allowed that will interfere with dancing. For example, no long necklaces, bracelets, dangling earrings, etc.
* Do not hang or swing from the ballet barres.
* Do not walk on the dance floor with dirty street shoes. Please help to keep the dance floor clean.
* Students are expected to treat the instructor and other classmates with respect at all times. The instructor has the right to ask a student who is misbehaving to sit out for the remainder of the class. No refund will be given for misconduct in class.
Absences, Add-Drop, Studio Closings

If you would like to add or drop a class, you must notify us in writing at email@example.com. You will be financially responsible for tuition up until the date that you notify us. If you have set up recurring payments and the new month has already been debited from your account, no refund will be given. For this reason, please notify us before the start of the new month in order to avoid the next month's debit.
In the event of a class cancellation due to inclement weather or for any other reason, you will receive an email and/or text and it will be posted on our Facebook page. If we have had to cancel more than one class due to inclement weather, we will schedule a makeup time for that class later in the season.
If you have to miss a class for any reason, you have the option to make it up in a different class if you would like. Please contact Miss Leslie at for the best time and class to make it up in.
Annual Recital & Aerial/Acro Showcase
Formations Dance Center has an Annual Recital (usually the weekend following Memorial Day). Any student participating in the recital must turn in a recital fee of $50 per family and costume fee of $55 per costume due the middle of November each year. They must also have the proper tights and shoes for each dance. As soon as recital fees and costume fees are paid, they are NON-REFUNDABLE. Admission to the recital is free.
We also have an Annual Acro & Aerial Showcase (usually March/April). This is an option for all students participating in an acro, tumbling or aerial dance class. Participants must turn in a costume fee of $55 in November. Admission to the show is $8 per person.
Trials & Private Lessons
If you would like to try a class before committing to the full season, you may do so for $10. You must contact Miss Leslie at to set up the day you are coming. If you decide to take the class, the $10 will be applied to that month's tuition.
Private lessons are $25 per half hour and can be set up with one of our studio instructors. Due to a full schedule, we may not always have the time or space available.
